Scheduling

Lakeside Medical Spa requires appointments to be booked. We encourage you to schedule appointments well in advance, especially prior to major holidays. Please note that upon scheduling your appointment, you will be asked to provide a credit card number to guarantee your treatment. Your card will not be charged until your appointment. Please refer to our cancellation policy prior to reserving your appointment time. Scheduling an appointment is your acceptance of this policy. We observe strict privacy policies and will not disclose this information to any other party. Your credit card number will be securely kept in your history file.

Consultations

We do offer free consultations if procedures are purchased at a consult appointment. If no purchase is made, you will be charged a stand-alone consultation fee of $50. If you arrive for a scheduled treatment appointment and decide not to receive any services, there will be a minimum charge of $50 or more, depending on services and amount of time scheduled, as this is considered a cancellation.

Appointment Reminder Policy

As a courtesy to our clients, we will send you text message and/or email reminders in the days leading up to your scheduled service. If you choose not to provide us with your cell phone number or email address, we are unable to offer you a reminder. Should the appointment reminder system fail for any reason and you do not receive an appointment reminder, it is still your responsibility to manage your appointment and adhere to the cancellation policy.

Cancellation & No Show Policy

Late arrivals will limit the time of your treatment, as your appointment will end at the scheduled time to accommodate the next patient's appointment. We will do our best to accommodate you; however, in some cases it may be necessary to reschedule your appointment. In such cases, the cancellation policy will apply. Cancellation Policy: Should you need to cancel, please do so at least 24 hours in advance of your scheduled appointment. If you cancel with less than 24 hours notice, your credit card on file will be charged the cancellation fee. If you do not arrive at your scheduled appointment without providing notification, the service will be considered rendered and you will either be charged the full amount of the service or the cancellation fee. The cancellation fee for all services is $50.

Service Refund & Product Return Policy

All treatments and packages are non-transferable and non-refundable and expire one year from the purchase date. Any unused services in your package will not be refunded. A refund, store credit, or product exchange may be given for any opened product that is returned due to an adverse reaction that a client has experienced while using that product. The client must consult with a technician prior to receiving a credit or exchange of the product.

Gift Certificates

Gift certificates are not redeemable for cash and cannot be replaced if lost or stolen. Gift Certificates are valid for one year from the purchase date.

Referral Policy

Referrals are greatly appreciated and are the highest compliment you could give us! To show our appreciation, patients are gifted a LMS VIP card with their first purchase of $300+, entering them into our referral program. Have referred parties show us a photo of your VIP card to receive a $50 discount, and a $50 credit applied to your personal account as well. It's our special way of saying thank you for spreading the love!

Parties and Group Bookings

We love hosting events! While Botox Parties are our specialty, we welcome parties of any kind. Parties that meet a minimum of 6 guests receive exclusive discounts on our products and treatments. Please contact us for details!

HIPAA & Privacy Policy

The HIPAA (Health Insurance Portability and Accountability Act of 1996) law allows for the use of information for treatment, payment, or healthcare operations. You have the right to restrict how your protected health information is used and disclosed for treatment, payment or healthcare operations. The providers and staff at Lakeside Family Medicine/Lakeside Medical Spa create and maintain private health records and other information describing, among other things, health history, symptoms, examination and test results, diagnoses, treatment, and any plans for future care or treatment. Patients are provided with a Notice of Privacy Policy that provides a more complete description of the uses and disclosures of certain health information. Patients have the right to review the notice prior to signing consent forms and/or receiving any treatment. The organization reserves the right to change the privacy notice and practices. Patients will be provided a new copy of the privacy policy notice with any significant changes prior to continuing service with Lakeside Family Medicine or affiliates.

Although we make every effort to keep our website and spa menu updated, please note that prices and services are subject to change at any time.

We love having you as a customer, but we do reserve the right to refuse service at any time, to anyone, for any reason.
